Hospice Registered Nurse (RN Case Manager)

Salary and Benefits
- Salary Range: $86,000 – $91,000 (based on hospice experience)
- Mileage & Vehicle Support:

- $36.92 bi-weekly car maintenance stipend

Responsibilities
- Full-time position; Monday to Friday
- Based out of the Marietta, GA office
- Manage a dedicated caseload of home-based hospice patients
- Coordinate care with a comprehensive interdisciplinary team
- Perform patient assessments and develop individualized care plans
- Provide clinical oversight and symptom management
- Educate patients and families on the hospice philosophy and support services
- Attend IDG meetings and contribute to team-based care planning
- Supervise LPNs and hospice aides when appropriate
- Maintain accurate and timely documentation in HCHB EMR system
Requirements
- Active RN license in the State of Georgia (unencumbered)
- Hospice experience is required (Home Health, ICU, and ER experience alone will not be considered)
- Current CPR certification
- Valid driver's license and reliable, insured vehicle
- Strong skills in clinical documentation, communication, and patient assessment
- EMR experience preferred (HCHB specifically)
- Must reside locally or be willing to relocate (due to traffic and territory coverage)
